PHP Script for Form with File Attachment

I developed contact us form for DagadTrikon.com using HiQFormMail PHP Script. It is available for download for free and allows file attachments. This is the best one I have found so far and is very easy to customize. Please note that this script will not work on Windows server. I hope you find it useful.

    I am working on modifying your form, but there is 1 thing I can not find… I would like to only have 1 file attachment, can you please let me know which file and what section of code I need to modify for that.

    For some reason this is not working, whatever I do it says: file attach failed invalid type of application – Doc1.doc

    I just want to configure it so that the person can attach documents… i.e. pdf, doc, docx, etc…
